If you would like to make a routine, new patient or follow-up appointment, please call us or submit an information request form. Please do not use the information request form for urgent questions or needs.

If you are a new patient or partnering agency that would like to refer an individual for mental health services, please use our referral form below. Completed referral forms can be mailed, faxed or hand delivered. Upon receipt of the completed referral form, QCI will contact you to schedule an appointment. You may also call our Referral Administrator who can complete a referral over the phone.
